In order to prepare for your initial consultation, here is what you need to know:
Please fill out the following form in as much detail as possible prior to your appointment.
This will help our team develop the best plan of action for your pet.
If your pet had a procedure such as IVDD surgery, TPLO, etc. we need written clearance from your pet's surgeon.
ALL REHABILITATION PATIENTS NEED AN INITIAL CONSULTATION WITH OUR MEDICAL DIRECTOR, DR. NATALIA NESTERENKO, DVM, CVA, CVSMT.
AT THIS TIME AN INITIAL EXAM WILL TAKE PLACE AND A REHABILITATION PROGRAM WILL BE ESTABLISHED BASED ON PATIENT ABILITY AND GOALS.
We will need all recent medical records and images prior to your initial appointment.
If you need assistance getting your pet's records, our office can assist you.
This information will help us understand your pet’s condition, evaluate prior treatments, and create a rehabilitation plan for your pet to live his best life.
Gathering Records
Please request a copy of your pets’ full medical records from your primary care veterinarian, including vaccine history, exam notes, and any laboratory results or radiology imaging. Records can be emailed to info@dawgrehab.com prior to date of appointment. We would also love for you to email us a picture of your pet that we can use for their records!
