In order to prepare for your initial consultation with our doctors, please fill out the following form in as much detail as possible prior to your appointment. This information will help your rehabilitation practitioner to understand your pet’s condition, evaluate prior treatments, and establish an idea as to your goals for physical therapy.
Gathering Records
Please request a copy of your pets’ full medical records from your primary care veterinarian, including vaccine history, exam notes, and any laboratory results or radiology imaging. Records can be emailed to info@dawgrehab.com prior to date of appointment. We would also love for you to email us a picture of your pet that we can use for their records!
